Highmark Funds SCHC's 2025 Expansion of Community Health Worker Program.

The Sussex County Health Coalition (SCHC)is proud to announce that it has secured funding to expand its Community Health Worker (CHW) Sustainability Pilot Program, an initiative to improve health equity in rural Sussex County, Delaware. This program, funded partly by the HHO Delaware HealthEquity Fund, will directly address health disparities, promote wellness, and create a replicable model for improving community health.

Healthcare inequities in Sussex County disproportionately affect marginalized populations, including individuals experiencing homelessness, addiction, and chronic health conditions, in rural areas, notably more than in metropolitan. The SCHC CHW program will be scaled to address these challenges by embedding a trained CHW within underserved communities, ensuring equitable access to healthcare for all.

The program aims to transform Sussex County residents' lives through improved health outcomes, increased access to care, a strengthened workforce, and enhanced collaboration. This initiative is not a local solution but serves as a model for statewide implementation. SCHC’s commitment to assessing the return on investment will guide future replication of the CHW program in other underserved communities to ensure equitability statewide.
